## Narrative

Employee stated EE was in the process of hanging fly pads in the #4 entry, on the Unit 2 Section. In the process of EE hanging a fly pad from the 6"" ladder, the ladder tipped over, and EE fell to the ground. This caused EE to land on EE's head, shoulder, and back. Note: EE was standing on the bottom 2nd step of the ladder when this incident occurred.
