Narrative

An unplanned roof fall was discovered in the Main North 6 entry at 190 block. The fall was approximately 16' wide, 80' long and 12' high. The fall was most likely caused due to gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.
