## Narrative

As employee exited a rubber tire yard forklift, foot missed or was partially on the step and they lost balance and fell backwards. Hard hat fell off and they struck the back of head on a vertical structural beam resulting in a 3"" long laceration. Employee was treated at the hospital with 3 staples to close the wound and a tetanus shot.
