## Narrative

At 5:40am an employee was dismounting the water truck at the end of night shift. While climbing down the steps and maintaining three points of contact, EE reached for the equipment shut off master switch. Footing slipped from the bottom step and they fell backwards from a height of 18"", to the ground resulting in injuries to the head and arm.
