## Narrative

Employee was using a 3/4 ton chain ratchet to replace return roller in tunnel. EE wrapped chain ratchet hook around piece of 8"" channel, after raising the belt EE bent over to put roller in, the chain came loose causing hook to hit EE in nose and mouth
