## Narrative

A fully trained miner was pulling miner slack using a center bolter. EE hooked a Roy rope to the bolter to pull the slack.  When tensioned the hook on the Roy rope spread and came off and contacted EE's thumb resulting in a laceration.  The injury was not the result of any compliance issues nor were any protective guards involved.
