## Narrative

At approximately 4:20 PM the western slope of the Old Coarse Refuse was discovered to have 3-4 ""hot spots"". These areas were smoking/smoldering with no visible flames. This was potentially the result of spontaneous combustion, there were not any problems with mining machines, PPE, training or compliance issues.
