## Narrative

Employee was grinding 2"" pipe when a metal flake entered left eye. EE immediately notified supervisor and flushed it with eye wash. EE was taken in for medical attention. The piece of metal had to be removed. EE was wearing safety glasses and a straight clear face shield. We will be changing over to a V90 shield with built in goggles to prevent this from re-occurring.
