## Narrative

On the night of 8/16/2018,the operator of the 628HT drove through a puddle causing mud to splash on the headlights and while traveling to the 4560 Q-Point the operator made contact with a berm on the left side. On 8/24/2018, the operator reported increased pain in their back.
