Narrative

Employee was getting onto Finish mill 4 w/ladder. Employee raised ladder it came in contact with a man basket causing dust get in EE's left eye. Employee was wearing standard issued safety glasses. Eyes were flushed on site, taken to clinic for medical evaluation, and given prescription eye drops. MEDICAL TREATMENT ONLY
