## Narrative

The employee was tightening a zip tie using a pair of pliers.  The pliers slipped off the zip tie and contacted the miner in the mouth causing a chipped tooth.  The miner saw the on-site medic and was referred to the dentist.  EE's tooth was fixed the next day and was issued a full release.
