Employee was untangling a 2"" rigid non-pressurized hydraulic hose when the hose slipped out of employee's hands and struck employee in the forehead causing a laceration that required 5 sutures. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
