An unplanned roof fall was discovered in the Old 6 North Mains #1 Entry at 11-12 block. The approximate dimensions of the fall are 60' long x 16' wide x 8-10' above the coal seam. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.
