Narrative

Employee was removing a nut from a bottom cradle. EE was using a hammer to remove the belt and EE's left index finger slipped and went between the hammer and the bed rail. This injury resulted in six stitches. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
