Employee was working at Lone Star leaching pad, cell 9 mod D. The employee attempted to cross a 12"" and 4"" pipe that was butted together by stepping over. EE stepped on 4"" pipe to get over 12"" pipe causing EE to slip and sprain EE's left knee.
