## Narrative

Employee was taking a pellet sample in the step II Agglom from line 4. While taking the sample a piece of slag hit the scoop knocking it out of employee's hand with the handle striking employee in the chin. Employee was sent to telemedicine and returned to work without restrictions but does state that employee has 2 broken teeth on the upper left side in the back.
