## Narrative

After unplugging a dust collector, a Production Assistant closed the system but the access door had been damaged so did not close properly at the top.  After the system was restarted, dust was blown out of the system and struck the employee in the face.  The employee visited the local hospital and was examined by an eye doctor, where employee was prescribed medication for eyes.
