Narrative

Employee was assigned to remove the cooler on a drill. The injured employee was holding hoses and wiring, guiding them through the frame of the cooler. There were two 2"" hydraulic hoses and one electrical conduit zip-tied together. As the cooler was being removed one of the hoses broke away and struck the injured employee in the face, chipping their left, front, upper tooth.
