The injured was installing a roof bolt in the #2 Entry of 9 East Section. EE had drilled into the hole enough to install EE's pusher onto EE's starter steel and as EE was inserting the steel into the chuck a piece of rock measuring 12"" L x7"" W x 1.5"" thick fell from the mine roof cutting off the tip of EE's right middle finger.
