Narrative

Employee was bending a 7' roof bolt to insert into a hole, when EE felt pain in the left shoulder. Employee has been treated numerous times for this injury with no resolve. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT UNTIL GOING OFF WORK ON 2/17/21 DUE TO SURGERY ON LEFT SHOULDER.
