## Narrative

The axle on the SP Material Hoist was changed as a preventative measure due to the sound the rebuilt axle was making.  The hoist was taken out of service, the axle changed, tested, and placed back into service.  MSHA was notified when the hoist was taken out of service and when placed back into service.
