Narrative

Employee slipped in the parking lot walking to EE's vehicle. EE struck head and strained back and neck. EE went to a chiropractor for treatment. Although EE was released for work, EE did not work on 2/11/21 due to inclement weather - snow and ice. EE did return on the 12th.
