Narrative

EMPLOYEE WAS TRYING TO PULL OUT A PARTIALLY INSTALLED BENT ROOF BOLT. EMPLOYEE LOWERED THE BOLTER HEAD AND THE BOLT SPRUNG TO THE SIDE STRIKING EMPLOYEE ON THE LEFT WRIST.
