On 3/11/21 A fall of ground occurred at 5000 East 21000. Was encountered when 1 of the night shift crew entered heading doing a workplace exam. Employee barricaded area & called supervisor. When supervisor arrived it was determined reportable to MSHA as it had propagated back into previously supported ground. MSHA hotline was called to report incident.
