## Narrative

The employees were removing a steel plate attached (welded) to the floor using a cutting torch and a fork lift.  The fork lift was under the plate and applying lift.  The other employee utilized a cutting torch to cut the final weld and when the weld was cut the plate  ""sprung""  up and landed on EE's foot causing two fractures in EE's left foot.
