Narrative

Employee was exiting a haul truck when foot slipped off the step causing employee's head to contact the door frame jarring employee's head & neck. Was seen at ER and given full release, at start of shift on 4/9/21 employee was still having neck pain and was seen a 2nd time by doctor and was given restrictions at this time.
