Employee stated that EE became nauseated on the way home after EE's shift on 04/27/2021. Employee sought medical attention at the Hospital. Employee was diagnosed as being dehydrated. Employee arrived to work 04/28/2021 and resigned. This request is being submitted and the matters described herein is not an admission to a reportable accident, injury, illness.
