Narrative

Employee had finished cleaning the discharge chute on the final conveyor and headed down the stairs. Due to poor lighting employee missed the first stair causing EE to fall. Employee then tried to catch themselves and slid on their bottom roughly 3-4 stairs until coming to a stop.
