Narrative

The employee had just completed pre-op and was handing it to foreman when a piece of rock measuring 3'x4' and from 10' to 2 ½"" thick fell from in-between a row of roof bolts. This rock struck the EE on the head pushing EE downward into the walkway of the roof bolt machine. EE's right hand was caught between the rock and the frame of the machine.
