The injured was bolting on 11 East #4 entry. EE was in the process of tightening (torqueing) a roof bolt when a McSweeney bolt wrench, that connects the drill head to the roof bolt, the wrench broke and fell from the top, striking them on the top of right foot resulting in a fracture to the 2nd metatarsal bone.
