Narrative

While attempting to bleed air pressure from a 6"" airline by loosening the coupler; the injured loosened the coupler once. Then while loosening the coupler a second time, the line blew apart striking the employee on the back of the head resulting in a laceration that required 3 staples. The employee was treated at the hospital and released to work full duty.
