Narrative

Ee was opening valve. EE stepped down onto hose causing the hose to unhook from the valve, and lime sprayed out, hitting EE in eyes. EE was transported to NNRH then to SLC for bilateral alkali burns to both eyes. Followed up with eye doctor on 7/19 and 7/21 and was put off work through 7/26 follow-up.
