Driver of Haul Truck informed me of back hurting early in the week of July 19 thru July 22. On July 20 was put on light duty. On July 21 they went to Urgent Care, doctor gave them a release for low back strain, with over the counter Tylenol and Ibuprofen and no work until the July 24.
