Narrative

The employee was on the #2 section in the #5 entry on the off side of the roof bolter. The Employee was attempting to install a glue bolt when a rock 15.5"" long X 4"" wide fell striking EE on the right arm just above the elbow. The area was approximately 7' high and damp. The laceration was approx 2 inches.
