Narrative

While assisting other employees with installing head drive guard; the chain hoist used to position the guard came loose striking the employee on the right side of the face. Employee was taken to a hospital where employee was examined and treated for two small lacerations to the face and chin resulting in a total of four stitches. Employee was released to work full duty.
