## Narrative

The washdown hose on the miner was coiled up on the rear of the machine with the valve end drooping over from the top of the machine to the floor. The employee was in the process of cutting coal when the frame of the machine caught the ball valve and broke it in two. The sudden release of water pressure caused the broken shut-off valve to strike the employee in chin.
