## Narrative

Employee stated, while making a belt splice on the stacker belt, the take up cradle rolled back mashing EE's left foot between the cradle and the frame. EE was transported via ambulance to ER where X-rays revealed broken bones in EE's left foot. EE was referred to a specialist. Specialist has removed from work and EE remains off work.
