Narrative

Employee stated EE was swinging the DDR drill pot over to install a roof bolt when a portion of draw rock measuring 14"" wide, 24"" long, 1.5"" in thickness fell from the mine roof and struck EE's left forearm causing a 3 inch laceration. EE was taken to a hospital where EE received medical treatment.
