## Narrative

While attempting to move a piece of curb of a conveyor with a 3/8"" grout chain the hook caught on the conveyor as the piece was being hoisted and the chain became tight and started to lift the conveyor. The operator stopped and placed a hand on the chain as it was being lowered and while the chain was still under tension, it dislodged from the conveyor and hit forearm
