Narrative

Employee was going to the east side of the building in the alley next to 12 conveyor to pick up the 936 loader. Employee walked around a puddle of water but there was a tiny bit of wet material about 1/8"" high. Material was slippery so when the employee walked over it they twisted right knee and fall to the ground.
