## Narrative

Miner removed bolts thought to secure ""cap"" protecting an oil sensor threaded and bolted into a pressurized tank. The cap was then dislodged from silicone seal causing the release of pressure inside the tank and cap to strike the miner in the jaw.  Other sensors had been accessed prior to this all located under bolted on protective covers not under pressure.
