The Employee was observing the bolt crew drill a test hole when a piece of rock approximately 3' L by 4' W By 1"" to 6"" thick fell striking EE on the hard hat, causing a 1 inch laceration on the back of EE's head. the entry was 18'6"" wide by 62"" high and damp to wet.
