Narrative

The employee was rib bolting on the return side of the 4 West Miner. When a piece of rock approximately 16""L x 4"" thick, fell from behind the tube striking the EE in the head and neck area. EE did not finish EE's shift and was taken for further evaluation. The EE was trained in the task.
