Narrative

While sitting on the bench to remove EE's sock the employee's foot slipped out from underneath EE. The motion caused EE to slip off the bench. EE caught self with EE's left arm on the bench causing injury to EE's shoulder. Evaluated at PMC and issued a full release with a shoulder strain. Follow up on 3/16/22 w/ Ortho. Diagnosed with torn shoulder muscles. Issued restrictions.
