A non-injury roof fall that measured 8' long and 8' high occurred on Spyder Panel 3, in the #4 entry at break 25. The fall was isolated to the #4 entry at the active pillar line. The release of supported roof was due to an overlying undetected separation above the anchorage of the primary support installed.
