IE was pulling 2 sleds with a scoop. Sleds were attached using (2) 6 foot 2"" dia. polyester slings wrapped around (2) 1"" eyebolts attached to the front corners of the tool sled. As the scoop was turning into a x-cut, this created tension on the R/S sling which caused that eyebolt to break. The broken eyebolt projected into the cab of the scoop striking IE on the R/S of the head.
