## Narrative

While installing insulation over a access panel, the employee realized it was upside down while on a ladder and was going to climb down, to turn it around. A gust of wind caught the insulation cover and caused EE to become unstable on the ladder. The employee jumped to the ground approx. 44"" and injured EE's left Achilles.
