Narrative

Employee was changing the left side ATRS jack on the 5W CM using a set of chain hoists to lift the stem out of the base. Employee failed to recognize the stored energy in EE's rigging and when the stem came out of the base it swung toward EE, striking EE's chin. Received a cut requiring 4 stitches. No rules or regulations were violated. Employee has training & experience.
