## Narrative

EE had to remove the bolts from the skid plate on haul truck to access the oil valve for the oil change. The manufacturer-provided chains were used to secure the skid plate up without bolts, but the chains failed. The skid plate fell approximately 4"" and struck the employee's right hand/wrist. EE sought medical attention the following day and diagnosis was a fractured wrist.
