Narrative

Ee walking to turnstile on the way in to work, slipped on ice, fell to ground. Seen at clinic, diagnosed low back strain, released with restrictions. Previously scheduled PTO resulted in no restricted duty days worked. Seen again on 11/13 c/o right wrist pain. Seen again on 11/14, xray negative, referred for MRI, released to full duty.
